Turbinate Reduction; Using a Cottle elevator, scalpel blade, or Ries suction elevator, an entry incision is made on the anterior face of the inferior turbinate to create a pocket. Alternatively, some surgeons have been using the PK zone on the blade to make the initial incision into the turbinate. This is done by setting PK to high and applying pressure to the entry point of the turbinate for a few seconds. After the incision, the soft tissue is then elevated from the turbinate bone. The PK SMR blade is then inserted into the pocket and is used to debulk the redundant tissue and/or bone and the PK is applied to achieve hemostasis. Dorsal venous complex (DVC) control during robotic radical prostatectomy can be performed by suture ligation (most commonly performed) as well as other hemostatic instruments. These include stapling and energy-based systems which are activated by the bed side assistant. Herein, we discuss the unique EnSeal technology for hemostasis and tissue welding. Furthermore, in a case video demonstration, we describe its use for a wide DVC which was not adequately controlled with initial suture. Rapid and efficient control was obtained in under 2 minutes and obviated the need for robotic instruement change.

Following prostate removal and ensurance of hemostasis, the reconstructive aspect of radical prostatectomy is performed. The anastamosis (the reattachment of the bladder neck to the urethra) is a delicate component of surgery. The robot surely helps in so far as the bloodless field, the magnified optics and the dime-sized needle drivers. Furthermore, with the use of continuous suturing (often 10-16 bites are taken around the urethral circumference), the watertightness of the anastamosis is far superior than the open anastamosis. Most open surgeons will only be able to place 4-6 interupted sutures and will often leave the foley catheter in longer than 5 days, as we do for robotic prostatectomy. During robotic-assisted anastamosis, 2 absorbably sutures (4-0 Monocryl)are used and begun their initial bites at the 6 o'clock position.
